Tip For June 1999: Emergency Contact Booklet
Do you take your day care kids to school and pick them up?
Do you have copies of your contact and consent forms with you in the car at all times?
You should, so here is how to put a booklet together yourself.
Materials:
Report cover with three ring tabs
Sheet protectors
Transportation forms and polaroids of the kids
Down load the transportaion safety forms from my web site. Have each child's parents fill it out and return it with a copy of the child's insurance information or card. Take a photo of each child. Arrange in the sheet protectors the forms, the copies if the insurance card and the photos. Write each child's name on the picture before you place it in the book. Clearly label the outside of the booklet. In case of an accident or other emergency, you and the emergency personnel have all the information they need to treat the children and contact the parents. If you have everything labeled clearly and you are unconcsious, they can still take care of the kids. It's a good idea to have a copy of your own insurance information and who to contact in an emergecy in there, too. Hopefully you will never have to use it.
